Corporate Critical Illness Claims Process

File your claim
Please complete and sign the Claim Form - Critical Illness with supporting documents to filing a critical illness claim
Send us your claim
Please send the Claim Form - Critical Illness with supporting documents to us by mail
All critical illness claims should be reported within 30 days from date of event to Liberty. Supporting Documents should be submitted to Liberty no later than 90 days from date of event.
Process your claim
Upon receiving all supporting documents, Liberty will process your claim.
Incomplete claim form cannot be accepted for processing of claim.
Further information may be needed.
Receive your claim amount
Reimbursement will be settled by cheuqe to the Policyholder. Notification of payment will be issued for your reference
