Term Life Claims Process
Each claim is different, let us guide you through your claim:
File your claim
- For life claim, please complete and sign the Personal Life Claimant Statement with supporting documents to filing a life claim
- For disability and accident claims, please reach our claims department for assistance at (852) 2892 3809 or Life.email@example.com
Send us your claim
Please send the Personal Life Claimant Statement with supporting documents to us by mail
All life, disability and accident claims should be reported within 30 days from date of event to Liberty. Proof of Loss should be submitted to Liberty no later than 90 days from date of event.
Process your claim
Upon receiving all supporting documents, Liberty will process your claim.
Incomplete claim form cannot be accepted for processing of claim.
Further information may be needed.
Receive your claim amount
Reimbursement will be settled by cheuqe to the Policyholder/Beneficiary. Notification of payment will be issued for your reference
